Contestant’s Application Form

IF YOU ARE READY TO SUBMIT YOUR APPLICATION, HERE ARE DETAILED INSTRUCTIONS AND GUIDELINES: Please appropriate some time to fill this out and have your credit card and portrait picture electronically handy. *We are currently using PayPal for all ticketing/ fee collection. The added transaction fees are included in the products listed on the registration page

Registration

NOTE:  Refunds (less fees) are only allowed within 2 weeks of purchase. Please contact us to request a refund. There are NO PARTIAL REFUNDS. We allow funds to be transferred to another participant’s fee. We can also apply the base fee for the following year.

  • Read all questionnaires and gather/ prepare your answers.  It is important that you provide your coach’s name and email information.
  • Read the Contestant’s Biography questionnaires and prepare your bio. See additional  instructions below.
  • You are now ready to fill out the application.  You would need to do this process in one seating, otherwise you may need to start all over.
  • Please save your answers in a PDF format or Text (copy/ paste method) so you have a copy in case the webpage disconnects and you lose your data. If you are unable to successfully submit, please email us the PDF file.  We are working out the quirks in this WP form.

INSTRUCTIONS FOR PICTURE SUBMISSIONS

  • Close up portrait photo (no action photos accepted)
  • JPEG format 150 – 300 dpi
  • At least 2” x 2” in size
  • Preferably Colored

INSTRUCTIONS WHEN SUBMITTING YOUR BIOS

  • Your narrative bio should be kept within 50 words or fewer.
  • Mandatory information needed:
    • Name of aerial/ circus school or studio you belong to
    • Name of your coaches/ mentor/s
    • How long have you been training in aerial arts
  • Optional:  Express who you are as an aerial arts artist.
  • You may NOT use the bio to promote a business or event.
  • We reserve the right to edit content to comply with above requirements
  • For Group Entrant (two or more),  we only need a single bio with  a photo of the entire group.  You are allowed no more than 100 words and tell your audience how you all came together, your inspirations and your shared visions.
